Why is もし added? What does it mean? Is it the same as in when you answer the phone?

In the first clause, the small つ is in the wrong place. 

もし、勉強しなかったら、

And, the second clause is not wrong but doesn't sound natural.  Since you're talking about a "possible future outcome, " it would sound better if you use ~くなる・~になる. 

せいせき が よくなくなる かも しれないよ。

Is this too hard to say?  Then, use the adjective わるい instead of よい.  How about this?

せいせき が わるくなる かも しれないよ

Also, when you want to ask if your example is correct, you can use a noun れい or たとえ in place of たとえば. たとえばis an adverb which means "for example."

私のたとえばはただしいですか Is my example correct?

 

’もし、勉強しなっかたら、せいせきがよくないかもしれないよ!If you do not study, your grades will be bad!
